IMPORTANT NOTE
If you wish to prevent your picture from being published to
the student directory, you must complete the FERPA Non-Disclosure
form and return it to us. You are not required
to re-submit the form after that unless you wish to change
your privacy status.
The Family Educational Rights and Privacy
Act of 1974, as amended, sets forth requirements regarding
the privacy of student records. FERPA governs:
- Release of these records (known as education
records) maintained by an educational institution and
- Access to these records.
Under the requirements of the law, schools
are required to notify students annually of their rights.
Student rights are as follows:
- Right to inspect and review their educational
records.
- Right to request to amend their educational
records.
- Right to limit disclosure of some "personally
identifiable information" (information that would directly
identify the student or make the students identity
easily traceable) known as directory information.
Directory Information includes:
student name
address
telephone listing
email address
photograph
major field of study
dates of attendance
degrees, honors and awards received
It is the policy of Saybrook Graduate School that your basic contact
information (name, email address, degree, concentration, photograph, and
biography) be posted in the SMS student directory - available only to registered
Saybrook faculty, students and administration. Should you not wish to have
your photograph posted in the SMS student directory, please print out the form using
the link above, sign and date it and return to:

*NOTE: Saybrook Graduate School will not release any information
to external agencies/individuals without the express written
consent of the student.
If this form is not received in the Office of the Registrar
by the beginning of the semester it will be assumed that all
information may be disclosed for the remainder of the academic
year. A new form for non-disclosure purposes must be completed
each semester.
